Credit card required to confirm your appointment
To keep costs down, Advanced Sinus requires that all new patients place an active credit card on file BEFORE their first visit. If you do not provide a credit card we will not be able to confirm your appointment.
Dr. Caballero is a sinus and allergy sub-specialist
She does not treat throat issues. She specializes in the treatment of sinus, nasal, and respiratory allergy problems.
She does NOT treat ear problems like ear infection, ringing in the ears, vertigo, or ear wax.
She does NOT treat skin rashes or food allergies. She is a respiratory allergist
Cancellation policy
If you book an appointment for food allergies, skin rashes, or ear/throat problems your appointment will be canceled and you will be responsible for the $100 cancelation fee.
Age restrictions
Her practice is limited to adults. You must be 18 to be seen. If you are older than 80 the doctor may need to be seen by her at the University of Chicago instead of the Park Ridge office.
